Eloralintide

Selective amylin receptor agonist · once weekly under the skin · the amylin half of Lilly's EloraTZP

Eloralintide is Eli Lilly's weekly amylin analog, a long-acting copy of the pancreatic fullness hormone. It favors the AMY1 amylin receptor 12-fold over the calcitonin receptor. People run it for weight loss. In a 48-week trial, 9 mg cut body weight 20.1%.

Selective Amylin Agonist · Appetite Control · Weight Loss · Once-Weekly

Eli Lilly's weekly amylin shot cut body weight 20.1% in 48 weeks on its own. It favors one amylin receptor 12-fold over the calcitonin receptor, and it stacks with tirzepatide as Lilly's EloraTZP.

The basics

  • A lab-made, long-acting copy of amylin, a pancreatic hormone that signals fullness.
  • Lilly built it to favor the AMY1 amylin receptor. Cagrilintide, the older amylin analog, is non-selective.

Why people use it

  • About 20% weight loss on its own. 9 mg a week cut body weight 20.1% over 48 weeks in phase 2.
  • Less nausea with a slow ramp. The 3, 6, 9 mg ladder had the least nausea of the higher-dose arms.
  • Steady all week. A 13–15 day half-life keeps blood levels nearly flat between shots.
  • Bigger numbers with tirzepatide. Lilly reported 23.3% loss on the top pair, versus 14.8% on tirzepatide alone, in adults with type 2 diabetes.

Stacks well with

  • Tirzepatide, the pairing Lilly tested as EloraTZP. Amylin adds a fullness signal on its own receptor, and the weight loss adds up.
  • Retatrutide, a pairing some people run at a low eloralintide dose for extra appetite control.

Good to know

  • Nausea ran 25% on the slow 3, 6, 9 mg ladder, against 33–64% when people started at 6 or 9 mg.
  • Fatigue is the other common one, 21–46% on the higher-dose arms versus 12% on placebo.
  • It clears slowly. After the last shot, levels take about 9 to 11 weeks to wash out.
Bottom line: A weekly amylin shot with about 20% weight loss on its own in phase 2, and Lilly's amylin partner for tirzepatide. People start low and step up every 4 weeks.

What it does

What the trials show for Lilly's selective amylin shot.

  • About 20% weight loss on its ownIn a 48-week human trial, people on 9 mg weekly lost 21.3 kg on average, against 0.2 kg on placebo.
  • Most people passed 10% lossOn 9 mg, 82.3% of trial participants lost at least 10% of body weight, against 12.9% on placebo.
  • Gentle at the starting dosesIn phase 2, nausea at the two lowest doses, 11% and 13%, stayed under placebo's 14%.
  • One dose already cuts weightOne 12 mg shot took off 4.4% of body weight by week 4 in healthy volunteers, while placebo gained 0.6%.
  • Results within 12 weeksIn a 12-week human study with no dose escalation, the 12 mg group lost 11.3% more than placebo.

02In plain English

How it works

Copies a fullness hormone. The pancreas releases amylin with insulin after meals. Amylin receptors are the calcitonin receptor paired with a helper protein called a RAMP.

Favors one receptor. In human cells, eloralintide was 12-fold more potent at AMY1R than at the calcitonin receptor and 11-fold more than at AMY3R. Lilly calls cagrilintide non-selective.

Lasts about two weeks. A C20 fatty acid on lysine 26 binds albumin, a blood protein, and a methylene bridge replaces amylin's disulfide for stability. The half-life is 13 to 15 days.

Acts through fullness. Lilly says the effect likely runs through satiety. A mild slowing of the stomach on day 3 of a 12-week trial had faded by day 80.

03Dose, schedule, cycle

How to dose it

A weekly shot under the skin, stepped up every 4 weeks.

Dose tiersThe usual start is 1–3 mg weekly, stepping up toward 6 or 9 mg.
Starting dose:
1–3 mg once weekly. Phase 2's 1 and 3 mg arms lost 9.4% and 12.4% over 48 weeks.
Every 4 weeks:
The slowest phase 2 arm ran 3 mg for 4 weeks, 6 mg for 4 weeks, then 9 mg, with the least nausea of the higher-dose arms, 25%.
Middle dose:
6 mg once weekly cut weight 17.6%. Started straight at 6 mg, it brought the most nausea, 64%.
Top dose:
9 mg once weekly, the top phase 2 dose. Weight fell 20.1% started at 9 mg and 16.5% on the 4-week ladder.
How to injectOne weekly shot into the fat under the skin, rotating sites.
Injection site:
Belly clear of the navel, outer thigh or upper arm, rotated each week.
Day and time:
One fixed weekday, any hour. See Best time to take it below.
Food:
Meals don't change an injected dose. Many eat small, low-fat meals on step-up weeks.
Cycle and breaksPeople run it continuously, as trials did for 12 to 48 weeks.
Course length:
Phase 2 ran 48 weeks; the phase 3 ENLIGHTEN trials measure weight at 64 weeks.
Build-up:
Each dose change takes about 6 to 7 weeks to reach 90% of its steady level.
Washout:
About 9 to 11 weeks after the last shot, five half-lives of 13–15 days.
Mixing the vialThe calculator defaults to a 10 mg vial with 2 mL of bacteriostatic water.
Default mix:
10 mg + 2 mL makes 5,000 mcg per mL, so each unit holds 50 mcg. 1 mg takes 20 units and 3 mg takes 60 units.
For 6 or 9 mg:
At 2 mL, 6 mg takes 120 units, more than a 100-unit syringe holds. With 1 mL, 6 mg takes 60 units and 9 mg takes 90 units.
Doses per vial:
A 10 mg vial holds ten 1 mg doses, three 3 mg doses, or one 6 or 9 mg dose.

04Timing

Best time to take it

Implied best time

Anytime (consistent, weekly)

Most people inject eloralintide on one fixed weekday. The hour does not matter.

  • Its 13–15 day half-life and slow absorption keep levels nearly flat. At steady state the peak ran only 1.28–1.38 times the trough.
  • Levels keep climbing for about 6 to 7 weeks after each dose change, so week-to-week consistency matters more than the clock.

05Popular pairings

What to stack it with

Another amylin drug such as Cagrilintide would hit the same receptors. The useful partners are incretins.

Eloralintide + Tirzepatide (EloraTZP)The pairing Lilly tested, as two separate weekly shots.
Why it works:
Tirzepatide works the GIP and GLP-1 receptors. Eloralintide adds amylin's fullness signal.
The protocol:
Both stepped up every 4 weeks, eloralintide from 1 or 3 mg to 3–9 mg and tirzepatide from 2.5 mg to 5–15 mg.
Adding it later:
Some participants added it at week 24 of tirzepatide, 3 mg for 4 weeks, then 6 mg.
Outcome:
Lilly reported 23.3% loss at 48 weeks on 9 mg plus tirzepatide 15 mg, versus 14.8% on tirzepatide alone, in type 2 diabetes.
The cost:
Lilly reported more stomach side effects and 10.8–27.0% quitting for side effects, against 2.9% on tirzepatide alone.
Eloralintide + RetatrutideA community add-on for more appetite control.
Why it works:
Retatrutide works the GLP-1, GIP and glucagon receptors but not amylin.
The protocol:
People report adding about 1 to 1.25 mg a week to retatrutide.
Outcome:
Reached for at a plateau. 1 mg is phase 2's lowest arm, which alone cut weight 9.4%.
Eloralintide + SemaglutideThe CagriSema idea, with a different amylin drug.
Why it works:
Semaglutide works GLP-1 only. Eloralintide adds amylin, as cagrilintide does in CagriSema.
The protocol:
Semaglutide stays steady; eloralintide goes 3 mg for 4 weeks, then 6 mg, as Lilly added it to tirzepatide.
Outcome:
ENLIGHTEN-6, a phase 3 trial, adds eloralintide for people on a stable weekly incretin.

06What helps it work

Support nutrients

Eloralintide curbs appetite, and its trials logged nausea, fatigue and constipation. These cover those costs and guard muscle on a deficit.

Settling nauseaGinger and vitamin B6 for the step-up weeks.
Ginger:
1–2 g ginger extract a day, before the first meal on injection and step-up days.
Vitamin B6 (pyridoxine):
25–50 mg daily. Long-term doses above 100 mg a day carry a nerve-damage risk.
Fatigue and electrolytesFatigue reached 21–46% on the higher-dose arms. Sodium and potassium are the first check.
Electrolytes:
About 500 mg sodium, 1,000 mg potassium and 200 mg magnesium a day, from food or supplements. Eating less drains all three.
Hydration:
2.5–3 L of water a day, since eating less also means drinking less.
Constipation reliefConstipation hit 24.1% on a fixed 9 mg, against 5.7% on placebo.
Magnesium glycinate or citrate:
300–400 mg elemental magnesium at bedtime, starting at 200 mg and building over 1–2 weeks.
Psyllium:
5–10 g psyllium husk in a large glass of water daily, away from other pills.
Protein, creatine and liftingProtecting muscle matters most when appetite drops this far.
Protein:
1.6–2.0 g per kg of body weight a day, protein first at each meal.
Creatine monohydrate:
3–5 g daily, no loading phase.
Resistance training:
2–3 sessions a week of progressive lifting.

Side effects and what to watch

What people notice. Nausea and fatigue lead. In phase 2, arms reaching 6 mg or more had 25–64% nausea and 21–46% fatigue, against 14% and 12% on placebo.

How people handle it. Starting low and stepping up every 4 weeks gave the least nausea of those arms.

Worth knowing. Phase 2 also logged slow heart rate (5.6–10.7% in three arms) and hair shedding (8–11% on arms reaching 9 mg), with none on placebo. In a 12-week trial, pulse fell 14.4 beats a minute at 12 mg, and four people on 6 or 12 mg reported low mood; the three who stopped recovered in 2 to 4 days.

Competing? It falls under WADA's S0 class, banned at all times.

09Quick answers

Common questions

How is eloralintide dosed?

One shot under the skin each week. The usual start is 1 to 3 mg, stepping up every 4 weeks toward 6 or 9 mg. The 3, 6, 9 mg ladder had the least nausea of the higher-dose arms.

How is it different from cagrilintide?

Both copy amylin. Lilly calls cagrilintide non-selective, while eloralintide favored the AMY1 receptor 12-fold over the calcitonin receptor in human cells. Its half-life is longer too, 13 to 15 days against about 7 days for cagrilintide.

Can I run it with tirzepatide?

That is Lilly's EloraTZP pairing, given as two separate weekly shots. In adults with type 2 diabetes, Lilly reported 23.3% weight loss at 48 weeks on the top pair, versus 14.8% on tirzepatide 15 mg alone.

Do I need to cycle it?

No. Trials dosed it every week for 12 to 48 weeks straight. After the last shot, its 13 to 15 day half-life means about 9 to 11 weeks to clear it.

How many doses are in a 10 mg vial?

Ten 1 mg doses, three 3 mg doses, or one 6 or 9 mg dose. Mixed with 2 mL of water, each unit holds 50 mcg, so 3 mg takes 60 units.
